Before Your Visit

Preparing for your first visit:

• Co-payment will be required at the time of your visit. The co-pay amount is variable, based on your health insurance plan. Payment is accepted in the form of cash, check or credit card (VISA, MC, DISCOVER).
• If required by your medical plan, a referral from your primary care provider must be obtained prior to your appointment. Please contact your health insurance provider to determine if this is necessary.
• What to bring to your appointment:
  • Current health insurance card
  • List of current medications and doses
  • List of all medication allergies
  • X-rays or other imaging studies pertaining to your orthopedic condition
  • Outside medical records (including previous operative reports and office notes) pertaining to your orthopedic condition
  • Completed New Patient Forms (these can be filled out by hand or electronically)
    • New Patient History

Subsequent visits:

• Unless you are within the 90-day global period after surgery, co-pay will be required at the time of your visit.
• What to bring to your appointment:
  • Current health insurance card
  • List of medication changes since last appointment
  • Any outside x-rays, other imaging studies, or medical records obtained since your last visit that pertain to your orthopedic condition
